Why Your Shoulder Clicks (And When You Actually Need to Worry About It)

If your shoulder clicks or pops every time you press, reach overhead, or go through your range of motion, you've probably wondered at some point whether something is wrong. The short answer: it depends entirely on what's causing it and whether it comes with anything else.

Shoulder noise on its own is one of the most common things active people mention in a clinical setting. Most of the time it's benign. But not always — and knowing the difference matters, especially if you're an overhead athlete or someone who trains consistently.

Why Shoulders Click in the First Place

There are a few distinct sources of shoulder noise, and they don't all mean the same thing.

The first is cavitation — the same mechanism behind cracking your knuckles. Your shoulder joint is filled with synovial fluid that contains dissolved gas. When joint pressure changes during movement, those gas bubbles can collapse and produce an audible pop. This is structurally harmless. The catch is that cavitation has a refractory period — once a joint cavitates, it typically takes fifteen to twenty minutes before enough gas redissolves into the fluid for it to happen again. So a shoulder that produces a single occasional pop is consistent with cavitation. A shoulder that clicks on every single rep of a set is not — something else is driving it.

The more common source of repetitive clicking is tendons or muscles snapping over bony prominences as the shoulder moves through its range of motion. The biceps tendon, the rotator cuff tendons, and the muscles around the scapula are all capable of producing audible movement as the shoulder goes through elevation, rotation, or pressing patterns. If this is the source, it's usually benign when painless — but it can also be a signal that something is off with the mechanics of how the shoulder is moving.

When the Labrum Is Involved

A third and more significant source is the labrum — the ring of cartilage that lines the shoulder socket, deepens the joint, and contributes to shoulder stability. When the labrum tears, the torn edges create loose tissue inside the joint that can flip and roll during movement, producing a deep clicking or clunking sensation that feels like it's coming from inside the joint rather than at the surface.

Here's what a lot of people don't know: a labral tear doesn't have to be painful to exist. In the early stages a torn labrum can produce clicking with completely normal function and no pain. That's exactly what makes it easy to dismiss. But if a tear progresses, the likelihood of developing pain, weakness, and instability increases significantly. What starts as a painless noise during overhead pressing can eventually limit your training, your throwing mechanics, or your ability to work overhead without symptoms.

The Question That Actually Matters

The presence of noise alone isn't the diagnostic finding. The question that changes everything is what comes with it.

A painless click with full range of motion and normal strength is usually not urgent. Worth monitoring, worth mentioning if it changes, but not a reason to panic.

A click or pop that comes with pain inside the joint, a feeling of the shoulder giving way or feeling unstable, weakness in overhead or pushing movements, or a loss of range of motion — that's a different conversation. Any of those findings alongside clicking points toward pathology that warrants a proper evaluation before it progresses further.

Rotator cuff involvement, labral pathology, bursitis, and early joint degeneration can all present with painful crepitus, and they require different treatment approaches depending on the source. Treating them all the same way — or waiting until the pain becomes severe enough to force the issue — is how minor shoulder problems become major ones.

The Bottom Line

Shoulder noise is common. Shoulder noise with pain, weakness, or instability is not something to manage on your own. If your shoulder has been clicking and you've noticed any of those accompanying findings — or if the pattern has been changing over time — it's worth getting a proper assessment to find out what's actually driving it before it becomes a bigger problem.
